1. If you would like to join the Kyushu Orthodontic Society, please click the membership application form button below and submit your membership information. Inconveniences may occur due to input errors on the membership application form, so please carefully check the contents before submitting.
2. After submitting the form, an email will be sent to your contact email address with instructions on how to deposit the membership fee and annual membership fee for the year. From there, please complete the payment procedures for the enrollment fee and first year's annual membership fee. Depending on your use of free e-mail or your mobile e-mail reception settings, you may not receive the automatic reply e-mail. If you do not receive an email with instructions on how to pay the membership fee and annual membership fee for the relevant year within one week after submitting the membership application form, please contact the Society's secretariat.
